Introduction:Treatments for major depressive disorder (MDD) are increasingly important for people with depression. However, the efficacy of antidepressants in improving mood has not been well-documented. Herein, we explore the potential of antidepressants to improve mood in people with MDD.
Aim:To assess the efficacy of antidepressants in improving mood in patients with MDD. Methods: This was a single-blind, randomized, placebo-controlled, parallel-group, open-label clinical trial. Participants with MDD who had a baseline Hamilton depression scale score ≥ 8 were randomized to one of two treatment groups: either an antidepressant (SSRIs, 5–7 days) or placebo (no treatment). A detailed description of the trial design, trial type, and participant characteristics are included in the manuscript.
Setting:The trial was conducted in Spain, a country with a relatively high burden of depression and health care services. The study was approved by the Research Ethics Committee of the Cantabria University (approval number: CEC-0824/2017-5). Participants were randomized to either placebo (n = 25) or an antidepressant (n = 25) according to their respective baseline Hamilton depression scale (HOMA) scores. Exclusion criteria included: (1) taking or using any antidepressants; (2) taking or using any other medication for depression; (3) taking or using any other treatment for MDD, including a combination of antidepressants and monoamine oxidase inhibitors (MAOIs); (4) taking or using any other psychiatric medication; (5) taking or using any medication for depression, including a combination of antidepressants and monoamine oxidase inhibitors (MAOIs); or (6) using any medication for depression, including a combination of antidepressants and monoamine oxidase inhibitors (MAOIs). Treatment was also defined as any antidepressant treatment (including a combination of antidepressants and MAOIs) in the previous 12 months. Follow-up:Participants underwent a psychiatric assessment at baseline. During follow-up, all participants underwent mood and depressive symptom measurements. Placebo-controlled trials are usually considered to have a high sample rate, with only 12 participants receiving placebo compared to 3,848 participants receiving an antidepressant. A total of 18,882 participants were included in this trial. The number of participants who were lost to follow-up was 9.8% in placebo-controlled trials and 6.2% in placebo-controlled trials. The number of participants who completed a follow-up period was 8.7% in placebo-controlled trials and 6.6% in placebo-controlled trials. Participants who completed a follow-up period were 1.6% less likely to be lost to follow-up (odds ratio [OR], 0.52; 95% confidence interval [CI], 0.32–0.94;P=.01).
Results:Among the 17,882 participants who were randomized, 18,882 (48.2%) met inclusion criteria. In comparison to the placebo group, the antidepressant group showed greater reductions in Hamilton depression scores from baseline (vs baseline: OR, 5.6; 95% CI, 3.2–9.5,=.002). There was no significant difference between the two treatment groups for Hamilton depression scores. There was no significant difference in HAM-D scores between the two treatment groups.
Conclusions:In people with MDD, antidepressants are effective in improving mood. However, there are limited data on the efficacy of antidepressants for improving mood in patients with MDD.
